Most people have heard of a Pacemaker. Far fewer know about its more powerful cousin, the ICD, even though an ICD is the one device in cardiology that can stop you from dying in your own Living room.

If your doctor has mentioned an ICD, or someone in your family has been advised to get one, here’s what the device actually does, who needs it, and what life with one looks like.

What an ICD actually does

An ICD (implantable cardioverter defibrillator) is a small metal device, about the size of a stopwatch, placed under the skin just below the collarbone. Wires run from it into the heart.

It watches every single heartbeat. When it detects a Rhythm that’s fast enough to be lethal, ventricular tachycardia or ventricular fibrillation, it delivers an electrical shock to the heart within seconds. That shock resets the heart’s electrical system and lets a normal rhythm return.

This matters because of how sudden cardiac arrest works: the heart doesn’t usually stop from pain or blockage alone, it stops because the electrical system falls into chaos and the heart can no longer pump. Without treatment, death follows within minutes. Even the fastest ambulance rarely arrives in time. An ICD is the only treatment that acts inside that window.

A pacemaker corrects hearts that beat too slowly. An ICD does everything a pacemaker does, plus it can shock a heart out of a fatal rhythm. Modern devices combine both functions.

Who needs an ICD?

Cardiologists recommend an ICD for two broad groups of Patients:

Secondary prevention — people who have already had a dangerous event:

Survivors of cardiac arrest

Patients who’ve had documented ventricular tachycardia with symptoms

Primary prevention — people at high risk who haven’t had an event yet:

Heart failure patients with an ejection fraction of 35% or below despite medication

People with scar tissue in the heart muscle after a large heart attack

Inherited conditions such as hypertrophic cardiomyopathy, long QT syndrome, or Brugada syndrome, where family hiStory may include sudden death at a young age

The ejection fraction number deserves attention. It measures how much blood the heart pumps out with each beat, and it comes from a simple echocardiogram. If yours is 35% or lower, ask your cardiologist specifically whether an ICD discussion applies to you. Many eligible patients in Bangladesh never get told, simply because the topic never came up.

What implantation involves

The procedure takes one to two hours under local anesthesia with light sedation. The cardiologist threads the leads through a vein under the collarbone into the heart chambers, positions them, then creates a small pocket under the skin and places the device inside.

Most patients go home after one to three nights. The incision heals in about four weeks, during which you keep arm movement on that side gentle. After that, most people return to Completely normal life.

What an ICD shock feels like

Patients describe it differently depending on whether they were conscious when it fired. Some compare it to a strong thump in the chest. Others say it feels like being kicked. It’s uncomfortable and frightening, but it lasts less than a second, and it means the device did exactly what it was implanted to do.

One important point: if you ever receive an ICD shock, tell your follow-up team, ideally the same day. One occasional shock is expected. Repeated shocks in a short period mean the device needs reprogramming or your rhythm condition needs additional treatment.

Living with an ICD

Daily life barely changes. A few specifics worth knowing:

Mobile phones and household electronics are fine. Keep a phone about 15 cm from the device rather than in a shirt pocket directly over it.

Airport Security: carry your ICD card, inform staff, and use a hand search instead of walking through the metal detector frame where possible.

MRI scans: newer MRI-conditional ICDs can be scanned safely with proper protocol. Ask for this feature if you’re choosing a device now.

Driving: Bangladesh has no strict legal restriction, but most doctors advise avoiding driving for the first weeks after Implantation, and longer if the device has already fired.

Follow-up: device checks every 3 to 6 months. Remote monitoring can handle many of these without a hospital trip.

Does it really work?

Yes, and the evidence is unusually strong. Large international trials over the past two decades showed that ICDs reduce sudden cardiac death dramatically in high-risk patients compared to medication alone. This isn’t an experimental therapy; it’s standard of care worldwide, including at major cardiac centers in Dhaka.
